chandelier illumination vitrectomy

Shinichi Kawashima, Motoko Kawashima & Kazuo Tsubota Vitreoretinal surgery has recently undergone remarkable developments. The introduction of micro-incision vitrectomy surgery using 25- and 23-gauge techniques has led to minimally invasive, faster, safer, and more accurate surgeries. Similarly, intraocular illumination and observation systems have advanced from light pipes to chandelier illumination, slit illumination, wide-angle viewing systems, and intraocular endoscopes. Compared to observation via the cornea and optic media, vitreous surgery using an endoscope is extremely useful, as the endoscope is inserted directly into the eye, circumventing the difficulties of corneal clouding and small pupil diameters. Furthermore, there are no blind spots after surgery. We here discuss the advantages of intraocular endoscope use, the current state of endoscopy-guided vitreoretinal surgery, and its future prospects.Keywords:: endoscopyeyemicro-incision vitrectomy surgerytechniquevitrectomy

Vitreous involvement as a result of penetrating trauma complicates and worsens the prognosis in retinal detachment repair. Seven patients in whom poor visualization of the fundus seemed to preclude success by routine procedures underwent combined vitrectomy, scleral buckling, release of subretinal fluid, and intravitreal gas injection.
chandelier taman seputehThe retina was successfully reattached in five of the seven patients, although postoperative visual acuity was limited - possible on account of the duration of retinal detachment prior to repair or because of the degree of macular involvement.

Part of the following topical collections:Diabetic Retinopathy: Medical and Surgical TherapiesAbstractThere have been significant recent advances in the management of diabetic tractional retinal detachments. Pre-operative intravitreal bevacizumab causes regression of active vessels and less intraoperative hemorrhage, small-gauge vitrectomy has dramatically changed how segmentation and delamination are performed, and bimanual surgery and viscodissection offer additional options. Intraoperative bevacizumab may decrease the incidence of postoperative vitreous hemorrhage. Single surgery success rates are generally >85 % with final anatomic success rates approaching 100 %.

Useful vision (VA ≥ 20/100) is attained in <40 % of patients in most series while ambulatory vision (VA ≥ 5/200) is attained in <80 % patients in most series. No light perception/light perception only vision occurs in <10 % patients while recurrent hemorrhage occurs in approximately one-third of patients. The Stellaris PC allows surgeons to have true “procedural choice” by providing the most advanced technology for both vitreoretinal and cataract surgery in a single system. It is a versatile system that allows surgeons the flexibility to perform different ophthalmic procedures while saving time, space and money for the surgeons and the patients they serve. Please note: The Stellaris PC replaces the MillenniumMicrosurgical System. Benefits to surgeons and their patients: It is the only vitreoretinal surgical system that comes with a dual light source and surgeon-selected color filters that allow for differentiated viewing designed to enhance the surgeon’s ability to see ocular tissue better under various surgical conditions. It allows surgeons a platform for the smallest incisions for both retinal and cataract surgeries. It is capable of 1.8 mm MICS phacoemulsification as well as a complement with Bausch + Lomb's existing proprietary 25-gauge system of instruments for retinal surgery, along with standard 23- and 20-gauge instruments.

It has light-weight, ultrahigh-speed vitrectomy probes for conventional and transconjunctival vitrectomy (TSV). Stellaris Meets the Needs of Surgeons Dr. Carl Awh, a long-time advisor to Bausch + Lomb and a private-practice physician with Tennessee Retina in Nashville, said, “The Stellaris PC meets the needs of today’s vitreoretinal and cataract surgeons. It allows us to perform the most advanced techniques with an intuitive, more compact system and is ideally equipped for both the ambulatory surgery center and the hospital operating room.”1.
